Preparing for your visit:
- Plan to arrive 30 min. (new patients)/15 min. (established patients) prior to your appointment time
- Have your current insurance card ready to show at check-in
- Log into your insurance website and make sure your insurance is active
- If your insurance requires a PCP (Primary Care Provider), make sure Dr. Michelle Daill is listed as the PCP
- Medicaid patients – we only accept Straight Medicaid, Meridian Medicaid, or UHC Community Plan. If your Medicaid shows anything other than these, you will need to have this changed prior to your appointment
- Have your co-pay ready if your visit is a sick visit
- If your address and/or phone number have changed, please update this at check-in
- If your account ha a past due balance, this balance needs to be paid at check-in
- If you are unable to keep your appointment, please contact our office at least 24-hours prior to appointment time. If you fail to attend your appointment, a $25 no show charge will be added to your account
