Preparing for your visit:

  • Plan to arrive 30 min. (new patients)/15 min. (established patients) prior to your appointment time
  • Have your current insurance card ready to show at check-in
  • Log into your insurance website and make sure your insurance is active
  • If your insurance requires a PCP (Primary Care Provider), make sure Dr. Michelle Daill is listed as the PCP
  • Medicaid patients – we only accept Straight Medicaid, Meridian Medicaid, or UHC Community Plan.  If your Medicaid shows anything other than these, you will need to have this changed prior to your appointment
  • Have your co-pay ready upon check-in
  • If your address and/or phone number have changed, please update this at check-in
  • If your account has a past due balance, this balance will need to be paid at check-in
  • If you are unable to keep your appointment, please contact our office at least 24-hours prior to appointment time.  If you fail to cancel your appointment outside the 24 hour period or you fail to attend your appointment, a $25 no show charge will be added to your account

Preventative & Yearly Physicals

Most health plans must cover a set of preventive services — like shots and screenings — at no cost to you. You will need to check with your insurance carrier to determine which services are at no cost to you.

Same Day Sick Visits

No need to visit the Urgent Care or ER, our office can provide same day sick visits for your child. Our hours are M - F 8:00 - 4:00 and are subject to change.

Medication Management

Whether your child suffers from allergies or has other health issues requiring medication, we can manage that medication for your child.

Sports & Camp Physicals

We can perform Sports & Camp Physicals when needed. You will need to check with your insurance to see how these exams are covered by your policy.

In-Office Tests

Our office performs in-office tests for RSV, strep, flu, & COVID.

Follow-up Visits

Our office will provide follow-up visits when your child has been seen by Urgent Care, After Hours Clinics or at the ER to ensure your child is receiving quality care.
